Consultation fees are due and payable at the time of visit.
Payment in full for surgery or other procedures a due and payable at the time of services unless prior arrangements have been made. If a patient cannot pay in full, then we will accept partial payment accompanied by an insurance from assigning benefits to our office or a payment plan can be arranged.
